    Description
    You will be responsible for specialist Continuing Healthcare assessments of individuals' health needs in various settings. You must manage the Fast Track process and complex care management to ensure all health needs are fully met.

    When assessing for Continuing Healthcare, you must provide written evidence to support your rationale for determining eligibility. You must act with care and compassion when planning care provision, respecting individuals' wishes and maintaining working relationships with colleagues from various disciplines.

    As a CHC Nurse assessor, you will promote the role as a multi-skilled specialist nursing position within the multidisciplinary team (MDT). You will support and teach nursing students on placement, maintaining a consistent standard of assessment. Additionally, you will undertake retrospective CHC applications and Rapid Deterioration processes as agreed upon.

    Main Duties

    You will ensure that individuals receiving packages of care can choose where and how they receive that care. You will allow people being assessed for health funding to have a voice, either directly or through representation by a chosen advocate. You will respect diversity, striving for equality in health and care delivery, and value every individual as autonomous, enabling them to make decisions about their life, even if those choices carry risk.

    • Undertake case management for complex clients to ensure high standards of care meeting their assessed needs.
    • Agree Fast track applications and ensure an appropriate Package of care is in place.
    • Monitor and report on the quality-of-care delivery in Nursing Homes, Residential Homes, and Domiciliary Care providers.
    • Implement the CHC process efficiently and transparently according to the National Framework.
    • Support acute and community hospitals with requests for discharge to nursing and specialist placements.
    • Promote close working relationships between the Local Authority and the NHS, disseminating good practice.
    • Provide specific advice and guidance on the National Framework to clients, their representatives, relatives, independent sector, NHS, and Social Care staff.
    About Us

    The Continuing Healthcare team comprises clinical and non-clinical staff, with an office base in Newton Abbot. Admin staff work full-time there, while clinical staff split their time between home and office bases.

    Qualifications

    Essential
    • Current NMC registration.
    • Nursing Degree or willingness to pursue this qualification.
    • Working towards HEAC342 Enhancing the care and support of the person with Dementia and their carer.
    • Teaching qualification - ENB 998/Mentorship/Practice Supervisor/Assessor or equivalent, or willingness to pursue.
    • Further professional or relevant qualifications at Diploma or Degree level.
    Knowledge

    Essential
    • Comprehensive understanding of the National Framework for NHS Continuing Healthcare, Funded Nursing Care, Joint Health and Social Care of the elderly.
    • General/mental health nursing knowledge and skills.
