Home Care Practitioner - 1 x 36hours per week
The role of the HCP involves working along side the community rehabilitation and nursing team. Care is delivered to individuals requiring rehabilitation input or/and palliative care.

The team delivers care to patients in their own homes, tasks can include; supporting with Activities of Daily Living i.e.

medication, personal care, catheter care, meal provision and support with nutrition.

The role will include provision of end of life support and care.

Care should be delivered in such a way to promote independence where possible and as part of an ongoing plan to support the patient to remain in their own home.

HCP support can be put into place at short notice to support discharges to hospital, prevent admission and for emergency situations in the community.
