Care Coordinator required - Bristol £22,600 - £26,000


Exciting opportunity has arisen for an experienced, organised and efficient Care Coordinators to join our team based in South Gloucestershire.


The role of the Care Coordinator is crucial to ensure the delivery of the highest quality care across all the areas we cover.

To be suitable for this opportunity you must have experience working within the care sector preferably as a supervisor or coordinator with willingness to learn and progress.


Details of the position.


You will work alongside a team of experienced professional senior care workers, field supervisors and managers to ensure effective and efficient running of the organisation.

Hours of work will be 9am - 5pm Monday - Friday + 1 week on call cover in every 4 weeks.


Responsibilities:

Plan and roster work on a weekly basis

Ensure rotas are organised and sufficiently well planned taking into account travel time and routes

Liaise with the relevant managers to ensure sufficient current and future staffing levels at all times


Work in partnership with all relevant Local Authorities, clients/their relations and all other key persons to ensure the smooth running of the organisation.

You will be required to support with office admin and dealing with day to day telephone enquiries.

Maintaining Service User and Care & Support Worker office files and ensuring that they are always up to date.

Ensure compliance with all office systems and procedures.


Experience & Requirements
Full drivers' licence & access to own vehicle

Must have Knowledge of rostering, call logging and call monitoring systems

Experience working within Adult Social Care, with a minimum of 2 years in a supervisory or Care Coordinator role.

Have practical experience of delivering care within the community.

Be able to build and maintain a good work relationship with others

Able to work on their own or as part of team

Have the ability to prioritise own work load

Excellent IT skills (Microsoft Word, Excel, Outlook)

Level 2 or 3 NVQ/QCF Qualified or willing to work towards.

Must be reliable, flexible honest & trustworthy

Excellent communication skills written, verbal and listening

Ability to be patient and keep calm under pressure.
