Description
The postholder will work under the direction and supervision of the Lead
Community Nurse or delegated Registered Learning Disabilities Nurse to assist
with the provision of and access to Primary Care for People with Learning
Disabilities, to contribute to a reduction in health inequalities.

THIS POST IS FIXED TERM/SECONDMENT FOR 6 MONTHS DUE TO FUNDING


If you are an internal applicant this post will only be offered on a secondment basis and you will need to have gained your line managers approval for the secondment prior to going to interview.

Work directly with Service Users providing practical support in accessing
Primary Care services, including access to an annual health check by the GP as directed by the Welsh Government.

Clinical
Work directly with Service Users providing practical support in accessing
Primary Care services, including access to an annual health check by the GP as directed by the Welsh Government.

Provide information to Service Users on health promotion and well-being.
Works autonomously with Service Users in community settings utilising a
person-centred and empowering approach, respecting the individuality, values and cultural diversity of Service Users.

Liaises with other services/agencies as appropriate.
Organises own caseload/workload including all administrative tasks and data entry. Records information related to the Service User in timely manner.
Reports any concerns or risks regarding Service Users to relevant staff.
Develops specialist knowledge of Primary Care resources within the area.
Demonstrates attitudes that ensure that the expressed needs and choices of the Service User are respected.
