Family Support Worker - Disabled Children's Team

Job Role
To provide care and support to children, young people and young adults with disabilities and additional needs.

The post holder will contribute to the assessment process, working flexibly across the Whole Life Disability Service to facilitate personalised support for disabled children, young people and their families, by offering direct support, practical help and advice.

This will include providing support to parents / carers in establishing day-to-day routines and fulfilling practical tasks that is based on a whole life, strength based, person centred approach, and plans being able to 'reach up' in working with children, youngpeople and adults as part of preparation for adulthood.
