An opportunity has arisen for an enthusiastic, personable and self-motivated individual to join the All Age Continuing Care as an Administrator.

This post will primarily support the Transforming Care Programme across Cheshire East and Cheshire West places.

This is a national programme aimed at improving the lives of children, young people and adults with a learning disability and/or autism who display behaviours that challenge, including those with a mental health condition.


  • Processing referrals.
  • Monitoring the generic mailbox and responding to queries.
  • Arranging appointments for patients, families / carers, and key health and social care professionals to attend.
  • Accurately inputting information onto the patient administration system.
  • Compiling data input submissions for NHS England.
  • Arrange and prepare meetings.
  • Taking notes.
You will play a key role in supporting the award-winning All Age Continuing Care service across Cheshire.

  • Acting as a first point of contact for the department, dealing with routine enquiries in a pleasant and helpful manner, communicating relevant information and referring to other team members as appropriate;
  • Providing a sensitive and efficient administrative support for the department and maintaining confidentiality of information relating to service users;
  • To book appointments following service standards. This will require having direct contact with distressed and anxious patients and their relatives, treating them with tact and dignity at all times;
  • Accurate and timely inputting onto department database, maintaining data storage and archiving of data as required and in accordance with procedures;
  • To arrange and prepare for Multi-Disciplinary Meetings, including the gathering and collation of relevant documentation;
  • Ensuring that all incoming post is collected and distributed accordingly and ensure that all outgoing post is correctly addressed and ready for collection at the designated time;
  • To handle complaints in accordance with standard operational policy, deescalating and resolving where possible and escalating to the appropriate person as required;
  • The job description and person specification are an outline of the task, responsibilities and outcomes of the role.   • All staff are expected to adhere and complete the NHS Continuing Healthcare Competency Framework on an annual basis.
