Description

Due to continued growth within our organisation, we are now looking to recruit two Care at Home Team Leaders to enhance our care management team.


Working as a member of the senior team providing care and support for service users, Plan safe working practices for the care team; providing leadership, instruction and supervision to all Home Support Workers.

Supervise the delivery of care, and ensure detailed care plans meet the individual needs of the service user, and that the quality of care meets the highest standards.


DUTIES

  • Monitor the service to Service Users according to Care Standards ensuring regular feedback of appropriate information to Service Manager/Care Supervisors/CEO, thus ensuring the safety and well being of the Service user and staff involved.
  • Deliver ongoing local formal and practical training to all team members to include specific topics as required by the care inspectorate and keep accurate and up to date records as evidence of all training undertaken.
  • To participate in SVQ process by carrying out the common induction skills training for new staff.
  • Supervise and monitor each member of the team and take appropriate action as required.
  • Carry out initial visits to service users to assist in the production of detailed service user plans and monitor and review those plans on a regular basis and as services users needs change.
  • Carry out detailed risk assessments and moving and handling risk assessments to minimise the areas of risk to the service user, staff, services providers and the general public, and to carry out regular reviews, ensuring information is shared with relevant parties.
  • Monitoring of the care worker as required in order to maintain personal skills in delivering home care / community support.
  • Maintenance, monitoring and record keeping in relation to attendance of care workers, service users care plans, risk assessments etc.
  • Undertake the duties of the Home Support Worker as required in order to maintain personal skills in delivering home care / community support.
  • To participate in the Out of Hours System to support staff and service outside of office hours. Utilise the Access People planner and care planning system to roster, allocate shifts, and implement care plans and reports.
  • Participate in team, multi-agency meetings and other meetings relevant to the post.
  • Maintain open effective communication channels within the service departmentally, and with other agencies.
  • Attend training courses as required and maintain own professional development and competency.
  • Work at all times within current Baillieston Community Care policies and procedures.
  • Work at all times in accordance with the Health & Social Care Standards.
  • Have knowledge of and work at all times within the requirements of Health and Safety regulations.
  • To maintain confidentiality of information acquired in the course of undertaking duties from the company.
  • Be willing to obtain appropriate registration with the SSSC within 6 months of starting employment, ensuring annual payment is made and appropriate qualifications are gained within agreed timescale.
  • Carryout any other duties related to the post at the direction of the Service Manager /CEO.

Licence/Certification:

  • Driving License (preferred)
