The Home First Bureau (HFB) Coordinator will support the HFB Matron/ Discharge Coordinator to manage the day to day operations of the HFB service at so that they provide a high quality, efficient and effective service that supports the safe transfer of patient care.


They will be required to ensure that processes are in place for the day to day running of the HFB supervision of staff and equitable allocation of work across teams and in all areas within their site.

The post holder will be the Single Point of Contact for the Community Resource Teams and Hospital Wards.

To support the timely and appropriate transfer of patients from Acute to Community Hospital, from hospital back to their own home ; from hospital to a new placement.

This will be an integrated role that will require cross boundary working with Health & Local Authority networks.


The post holder will ensure that best practice is adhered to and actively monitor the quality of products and services that are provided e.g.

validating referrals, ensuring timely communication with wards and CRT and ensuring data is correct, live and available to appropriate parties.

The HFB Coordinator will be required to deputise for their discharge coordinators.

As part of the HFB team they will be required to assist with the delivery of the Health Boards Discharge to Recover and Assess Strategy across Gwynedd & Mon (D2RA) by leading on small elements of it or assisting others to deliver their lead areas of work.

The ability to speak Welsh is desirable for this post; English and/or Welsh speakers are equally welcome to apply.
