An exciting opportunity has arisen for experienced individuals to join our Integrated Discharge Team as a Patient Flow Co-Ordinator. Are you passionate about ensuring patients are in the right place and supported to the right pathway?


Shropshire Community Health Trust is offering an exciting opportunity for motivated individuals to work within the Integrated Health and Social Care Discharge Team at the Shrewsbury and Telford Hospital Trust.

The team is expanding, and this is an ideal opportunity to be involved in the transformation of the service and work with the existing skilled team in the specialist field of Complex Discharge.

The successful applicants will be key members of the multidisciplinary health and social care Integrated Discharge Team.


The Capacity Hub Patient Flow Co-ordinator will be responsible for triaging and co-ordinating flow/ admissions to our community hospitals and independent sector beds and all associated and administration functions.

Manage and maintain system and trust service reports. Maximise optimum bed occupancy and support hospital flow discharge from Acute to Community Services.


The post holder will co-ordinate and navigate the patient across pathways to ensure timely discharge for patients deemed fit for discharge.

The role and service offer integration and partnership working with team managers, nurses, therapists, health and social care coordinators across both organisations to ensure a seamless service


The service operates 8am - 6pm, 7 days a week, this post includes bank holidays and weekend working as part of the clinical team rota.


Shropshire Community Health NHS Trust provides community-based health services for adults and children in Shropshire, Telford and Wrekin, and some services in surrounding areas too.

These range from district nursing, health visiting and running four community hospitals through to providing very specialist community care through talented and dedicated staff.
