Job Description:


  • Support the Registered Manager to forward plan the care schedules to ensure continuity of care.
  • Be responsive to changes in the schedule and liaise with the relevant team members.
  • Prepared to work variable hours when required.
  • Ensure effective communication with external sources such as next of kin, GPs, nurses, occupational therapists and any other professional if their involvement impacts on our client.
  • Ensure that all Care Professional holiday, sickness, unplanned absence and all emergency calls are documented.
  • Conduct client and Care Professional introductions.
  • Cover care calls and gaps in the care schedules where they arise and cannot be covered.
  • Support and mentor Care Professional
  • Maintain regular contact with clients and Care Professional in order to develop excellent relationships.
  • Any other duties as directed by your line manager.
  • Participate in the operations team oncall duties as required on a 5week rota.
  • Ensure all care plans and other documentation, including personnel files are compliant with regulators and internal audit standards.
  • Carry out quality assurance reviews with our clients to ensure we are delivering outstanding service and record.
  • Ensure compliance with Home Instead's Equality, Diversity and Equal Opportunities Policy, in respect of employment and service delivery.
  • Carry out any other duties deemed necessary for the successful operation of the business.
  • Understand the arrangement for ensuring all clients are safeguarded against the risk of abuse and respond appropriately to any allegations of abuse.

Qualifications:


  • To work collaboratively in a team to be mutually supportive.
  • Level 3 NVQ in Health and Social Care or equivalent is preferable.
  • Experience of coordinating rotas or schedules is desired, but not essential.
  • Experience in the care sector delivering a wide range of personal care services.
  • Preferred experience with a CRM system, we use People Planner
  • Passion for delivering the highest quality care to our clients and helping them to live independently and happily in their own homes.
  • Knowledge and understanding of legislation and regulations specific to Health and Social Care.
  • Good communication skills with the ability to build rapport quickly.
  • Good working knowledge of IT systems with experience of Microsoft Office and virtual communication platforms with the aptitude to learn and adopt new technologies and software where appropriate.
  • Must have full driving license and means of transport if required within the territory to visit clients.
  • Be organised and flexible to meet the needs of the business.
