We are seeking to recruit a Social Work Assistant to work with the Supported Living, Learning Disabilities Team at Market Road Resource Centre, Canton.

This post is part of an ongoing, exciting development to enable adults with learning disabilities to live closer to home.

It is an ICF (Welsh Government) funded project to support the development of regional services across Cardiff and the Vale.

This position is a 37 hour week position Monday - Friday. You will be based both on site in Canton and are home.

The post holders will become part of the Supported Living Team and will be responsible for supporting the work of the team by:

Carrying out Monitoring Visits to Supported Living Schemes
Assisting with the overall Monitoring of the Supported Living Contact
Being part of an enthusiastic, dynamic and evolving service
Being part of planning and facilitating moves for individuals
