Job Summary:The North Riding Community Health Primary Care Network are seeking to recruit a digital care coordinator to be based at Derwent Practice, Malton to work within the existing practice Data Quality team.

Derwent Practice is a friendly, family GP Practice based in the heart of Malton, North Yorkshire. The Practice is made up of 10 GP partners and holds a list size of 21,000 patients. The practice was rated 'Good' overall by CQC in May 2023.


They will work as part of the existing Data Quality team under the direct supervision of the Data Quality Manager, ensuring that they provide a high-quality service at all times, adhering to practice protocols and national guidelines in keeping with patient safety.


They will be an integral part of the practice team, providing high quality assistance undertaking a variety of administrative duties to assist in the smooth running of the Data Quality team.

North Riding Community Health Network was founded in 2019 and has four member practices.

We are located on the edge of the North York Moors National Park and our member practices are Ayton and Snainton Medical Practice, Ampleforth and Hovingham Surgeries, Derwent Practice and Sherburn and Rillington Medical Practice.

We provide care to an overall patient population of 39,000.

Each member practice has a CQC rating of good and 3 out of the 4 practices are rural dispensing practices.

Our aim is to deliver high quality services for our patients, facilitated by working collaboratively, to better respond to the needs of our local population.

PRIMARY DUTIES AND AREAS OF RESPONSIBILITY

  • To undertake a variety of administrative duties to assist in the smooth running of the Data Quality team, working under the direct supervision of the Data Quality Manager
  • To work within set parameters to ensure documents have been uploaded to the correct patient records and are legible.
  • To assist in appropriately coding/extracting relevant information from clinical correspondence to file on patient records.
  • To manage documents and action any documents in adherence to practice procedures.
  • To ensure that documents are processed in line with department protocols and to the highest standard in keeping with patient safety.
  • To effectively use the computer software provided
  • To carry out efficiently all associated tasks to support the smooth running of the department that may be assigned to you by a supervisor.
  • Work collaboratively with other teams and services to maintain an effective and efficient service to patients.

  • Information relating to patients, carers, colleagues, other healthcare workers or the business of the Practice may only be divulged to authorised persons in accordance with the Practice policies and procedures relating to confidentiality and the protection of personal and sensitive data.
  • A Disclosure and Barring Service (DBS) check (previously CRB checks) will be undertaken as a matter of course.

Learning and Development:

The post-holder will participate in any training programme implemented by the Practice as part of this employment, such training to include:

  • Participation in an annual individual performance review, including taking responsibility for maintaining a record of their own personal and/or professional development.
  • Taking responsibility for their own development, learning and performance and demonstrating skills and activities to others who are undertaking similar work.
  • Undertake mandatory and statutory training as required.
