Description

We have an excellent opportunity for a Social Care Practitioner to join our busy, but supportive generic adults, team covering the High Peak North area of Derbyshire.

We are based in Glossop in the High Peak and our Team consists of Social Workers, OT's, Social Care Practitioners, a Senior Practitioner, and a Service Manager.

We are a busy team, working predominantly alongside adults over the age of 18, with the exception of working with younger adults, as they approach adulthood.

We are looking for an experienced practitioner, with exceptional knowledge and experience of working with the Care Act (2014), alongside supporting legislation i.e.

, Mental Capacity Act

We provide a wide range of services to our community, including Care Assessment, support plans, advocacy, safeguarding duties, to name a few.

We are enthusiastic about a strength-based approaches and outcomes.

In the coming months we are about to embark improved integrated ways of working, with health colleagues taking a collaborative approach in supporting our community.
