Job Summary
To address the ever-increasing demand for health care services and to better support individuals within their own communities, there is wide recognition of the need to embrace new ways of working and digital healthcare technologies to ensure all patients receive the right care, in the right place, at the right time.
Digital care coordination is a mechanism for delivering 21st Century care and connecting people with both medical and non-medical sources of support, empowering people to better self-manage their conditions and/or work collaboratively with healthcare professionals to manage their conditions.

Duties and responsibilities
- To work closely with the project team in the deployment of new technologies and education of patients.
- Work with patients to improve digital inclusion.
- To act as an ambassador for Digital healthcare technology with patients and colleagues in equal measure
- Be confident in actioning administrative tasks in relevance to digital projects
- To run specific reports for review and monitoring, as directed to meet the requirements of contracts within the Primary Care Network. To identify patients who may benefit from new services/new ways of delivering services and ensure appropriate referrals are made
- Act as the first point of contact for digital enquiries
- Demonstrate and educate colleagues about how to optimally use digital technologies and data and intelligence
- Work with partner organisations to promote digital inclusion.
- Troubleshoot for colleagues and patients
- Ensure knowledge and skills are up to date by attending training events and webinars.
- To participate in the promotion of Shore Medical via events, notice boards, producing information for web sites and social media platforms.
- Where licences are required, take the lead in releasing licences to patients and colleagues in line with implementation plans
- Work as part of the local multidisciplinary team pulling colleagues together around the common opportunities from digital and new ways of working
- Implementing data quality assurance on all practice activity data.
- Ensures that all H&S policies are followed and aware of the personal responsibilities in relation to Health and Safety in the workplace
- To follow all company policies and procedures
