Remote Monitoring Hub Clinical Lead Practitioner - Band 8a

We are developing the services that we currently provide to reduce hospital admissions and to facilitate earlier discharge from hospital through the use of technology, ensuring patients have the safest best care that is delivered in their home environment via the Croydon Remote Monitoring Hub (Virtual Ward).

The Virtual Ward service available 24 hours a day, 7 days a week on site.


You will act as an expert and collaborative advanced practitioner who will provide advice, education and support for patients, families and members of the multidisciplinary healthcare team.

A key emphasis of the role is to empower staff to provide best practice for patients and to work as an effective role model.

You will work autonomously with a patient caseload holder within the SWL remote monitoring team (virtual ward). The Virtual Ward Clinical Lead Practitioner will provide a pathway to diagnosis and management. As an advanced practitioner appropriate referral for investigations, treatment and management will be facilitated and initiated.


The post holder will frame their practice within the four sub roles of expert practice, professional leadership and consultancy, education, training and development, practice/service development, research and evaluation.


The Virtual Ward Clinical Lead Practitioner will support and enhance partnership working across secondary care, community and primary care teams to optimise the services that our patients can access.

They should also consult at an expert level on the central and local MDT clinics.

The post holder should represent the virtual ward at regional and pan London networks where appropriate to ensure up-to-date delivery, promote the virtual ward services within the sector and engage in shared learning.

To provide a Virtual Ward Clinical Lead Practitioner expert role within the virtual ward hub, which supports and develops practice upon clinical expertise and evidence, actively promoting quality issues and governance in all areas of work.


  • Ensuring that development opportunities are available for all staff, acting as a mentor and role model promoting an open and fair culture throughout the Virtual Ward.
  • Must have excellent communication and interpersonal skills.
  • Ability to negotiate and interact with key stakeholders on clinical issues and service development.
  • Demonstrate expert practice with clinical credibility essential in all caseload areas and with various professions and agencies that interact with the Virtual Ward Operational Lead.
  • Providing professional advice, support and excellence in clinical care of patients with acute and chronic conditions that can be managed remotely using virtual ward remote monitoring.
  • Promoting health and wellbeing, actively supporting the development of patients in enabling them to become experts in managing their conditions to a level they find appropriate.
  • To provide nonmedical prescribing support, prescribing with responsibility, safety and within clinical knowledge limitations.
