At Live Well Leeds you will be part of a strategic partnership led by Touchstone, in collaboration with Community Links and Leeds Mind.

Live Well Leeds delivers flexible and practical support to individuals aged 18+ across the Leeds District. As a Mental Health Support Coordinator, you will be responsible for co-producing goal-oriented support plans with service users and conduct regular reviews to ensure the best service possible

**If you have: - Experience in mental health support or related field.

  • Strong communication and interpersonal skills.
  • Ability to work collaboratively within a diverse team.
  • Commitment to coproductive approaches and person centred care.
