Overview:
We are seeking a highly organized and detail-oriented individual to join our team as a Care Coordinator.
As a Care Coordinator, you will play a crucial role in ensuring the smooth and efficient day to day running of our care settings.
This is an exciting opportunity to contribute to the success of our company and work in a fast-paced environment.Responsibilities:
- Represent the company in any relevant meeting, including social workers, parents and other professionals along with the Registered manager.
- Supervise the processes involved in the care of the service users, within the context of company policies and procedures, The Care Act 2014, and any other relevant legislation including CQC and commissioning authorities' requirements.
- Take part in and contribute to reviews of the care provided at team meetings, external reviews with family, social work team, medical professionals, etc.
- Contribute to individual service users care including care planning and risk assessments alongside the Registered manager.
- Participates as a member on the management team, with the Registered Manager and the Directors to ensure high standards of service delivery and effective communications with all people involved with Crystal Care and Support Ltd.
- Support staff members to deliver a high quality service to the individuals, including undertaking supervisions and observations and championing best practice.
- Plan activities for service users to ensure individuals needs are met and that meaningful activities take place.
