The Quality and Patient Safety Team is looking for a committed and innovative individual with experience across all areas of quality and safety to join our team as the Patient Safety Investigation Lead.

This is a brand new role, heading up our new Patient Safety Investigation Team.

This is an exciting time to be joining an enthusiastic and collaborative Quality and Patient Safety team, who are working to deliver safe and
effective care to our patients. We believe PSIRF offers a real opportunity to improve the way we investigate and learn from incidents, and this
post is a key role in making this a reality.


The Patient Safety Investigation Lead will oversee the team of people responsible for investigating and reviewing incidents on behalf of the Trust.

They will lead the Patient Safety Incident Investigators in supporting patients, families and staff through the difficult process of an investigation.


The post holder will ensure our investigations into patient safety incidents and other incidents are thorough and high quality, using dedicated learning responses and ensuring Being Open principles are applied.

The post holder will support the Patient Safety Incident Investigators and Patient Safety Team in categorising and classifying patient safety incidents and other incidents requiring investigation to assist in the identification of trends.
