North East London Foundation Trust have an exciting opportunity for a Nurse to join the Care Closer to Home team in Waltham Forest

The post holder will work as a Care Closer to Home Operational lead Nurse, supporting the MDT to assist patients to live well in their homes.

You will be involved in delivering integration, working with GP's, Social services and the third sector organisations.


You will be providing operational management, case management, facilitating early discharge from hospital and preventing unnecessary/unplanned hospital admissions where appropriate as well as supporting healthcare closer to home.

The service currently operates from hrs, Monday to Friday.


The post holder will be expected to support their team, department and organisation to achieve the Trust's Values in their day to day work.


The post holder will be an experienced Nurse registered with the NMC with at least 3 years community experience of managing a complex caseload of patients.

You will be responsible for a day to day management of the team, and a caseload of patients with complex health and social care needs.

You will work autonomously to case manage these clients and use advanced therapy skills, such as assessment and prescribing to manage their conditions in the community.

He/She will work as part of the integrated care team, working within and across professional and organisational boundaries. Liaising with GPs, other professionals and statutory and voluntary agencies he/she will deliver individualised care to patients.

Probationary Period

Starting with NELFT
NELFT place a great deal of importance on new starters being properly welcomed and inducted into the Trust.

All new starters will join the Trust on the first Monday of each month and will undertake a comprehensive induction of up to two weeks which will include mandatory training, systems training and the allocation of equipment.

As part of the process new starters will have the opportunity to also meet the executive team, senior managers and attend a number of drop in sessions focusing in engagement, health and wellbeing and key processes.

The induction will be held at our head office in Rainham, Essex.

High Cost Area Supplement - London

This post also attracts payment for High Cost Area Supplement of 15% of the basic salary (minimum payment of £4,108 and a maximum payment of £5,177 per annum pro rata).


This role requires an individual who can provide strong leadership to secure the delivery of highly organised, well planned and effective discharge planning.


The successful post holder will support the development of strategic direction for the services in line with national policies and lead on continuous improvement and service development.

You will help to create a culture in which innovation can flourish and provide vision and inspiration to enable staff to deliver high quality services and patient care.
