We have an exciting opportunity for a motivated, enthusiastic and responsible Health Visitor SCPHN to work within our Growing Healthy Service in Darlington.
We are a skill mixed team of 50, with two teams working across Darlington.We are a enthusiastic forward thinking team with a outstanding performance record in caring for children and young people living in Darlington.
We work in partnership with Midwives, Early Help, Children's Services, Public Health and Acute Services.We have a hybrid approach to working giving you the practitioner autonomous flexibility.
There is a large Office within the Beehive in Darlington with a onsite admin and management team, the team pride themselves on their kindness and approach to team working.
Any new members of the team will receive the customary Darlington warm welcomeNew team members are supported through induction, training, shadowing and a preceptorship package tailored to your individual needs.
- 46 weekly supervision with Line Manager and Safeguarding Team
- Monthly Preceptorship/ Peer Supervision
- Appraisal in the first 3 months of the role and then annually
- In addition to this have a wellness champion and wellness Wednesday events running.

Utilise specialist skills and knowledge to assess the needs of clients and their families and Communities. Develop, deliver and review packages of care that ensure appropriate treatment and referral to maximise potential health outcomes.
Work as an autonomous practitioner, assuming professional and legal accountability for all aspects of own work in line with the Trust's objectives and national and local guidance.
Utilising caseload profiling, identify the health needs of a designated area and promote programmes for the client population to alleviate the problems caused by physical, social and emotional ill health.
Liaise closely with other health professionals, statutory and voluntary agencies, in order to ensure a cohesive plan of care for individuals, families and communities is delivered.
Provide day to day management and supervision to support staff in delivering an agreed programme of care.
Ensure complete accurate and contemporaneous records in accordance with the N.M.C and Trust guidelines, policies and procedures.
