Description

We are looking for an experienced mental health professional (nurse, social worker or occupational therapist) to join the Recovery and Wellbeing Team based in Castle Point.

We are a dynamic, multi-disciplinary service, providing a range of specialist interventions using an integrated person-centred approach to adults who are residents of the Castle Point area.

We work with those who have a diagnosis of a mental health condition and often have complex presentations.

These can include illicit substance/alcohol use or physical health concerns and sometimes be further compounded by complicated familial and/or social dynamics and factors.

You will be expected to, assess, plan, implement and evaluate all aspects of care.

You will be part of a vibrant and experienced multi-disciplinary team and possess an understanding of how multidisciplinary teams need to function.

You will also be expected to liaise with other statutory and non-statutory agencies in the provision of care and have the ability and confidence to engage service users, carers and relatives with empathy and compassion whilst working towards an individual's recovery goals.


The team's operational working hours are Monday to Friday 9am to 5pm and the team base is in Canvey, although at times other trust bases/alternatives can be used depending on the situation and team priorities.


  • To manage a caseload of mental health serviceusers with complex needs as lead professional and care coordinator under the CPA process.
  • To use evidencebased and clientcentred principles to assess needs and to plan, implement and evaluate health and social care interventions.
  • To facilitate flow through the service and timely stepdown to low intensity services and primary care.
  • To create and continually update personcentred care plans and build therapeutic relationships with your clients.
  • To assess risk and design personalised risk management plans.
  • To provide leadership for junior staff through supervision and appraisal.
  • To participate in the planning, development and evaluation of MDT services, holding responsibility for defined projects and allocated tasks.
  • To mentor and supervise students on practice placements.
  • To carry out MDT activities e.g. coordinating meetings and undertaking duty responsibilities.
  • To identify any possible safeguarding concerns and be responsible for the completion of safeguarding investigations.
  • To ensure that the requirements of the Mental Health Act 1983 are observed, adhered to and implemented.
  • To support the promotion of carer and service user involvement within the service and for its provision plus ensuring carer's needs are identified and responded to.
  • To provide professionspecific interventions for the wider team as required and in line with NICE guidelines.
