Registered Mental Health Nurse
Opportunity available for a Registered Mental Health Nurse to work as a care coordinator within the community, Based in Aylesbury

The team sits within a health centre with the role predominately acting as CPA Care Coordinator and manage a caseload of clients with varying mental health diagnoses.

As part of this role you will be completing risk assessment and client
- centred care plans and providing face to face treatment in the community to these clients and conduct a 'duty' role on a rota basis, responding to the needs of clients who need some additional support. You will maintain all aspects of client records in line with best practice and has experience of working with people with mental health problems. It is essential you are a qualified Registered Mental Health Nurse with a Valid NMC Pin.

This role is to commence ASAP with the role being full time working 37.5 hours per week with shifts being Monday to Friday, 9am to 5pm

Hourly rate of pay:
£28 Per Hour
