Description

Have responsibility for sharing outcome data for service development and quality improvement. Work as the key link between system service providers delivering aspects of the system flow hub. Direct and indirect line management responsibilities within the system UEC team. Advise, assist and support with planning for development of discharge pathways and admission avoidance processes.

Develop strong links and networks with partner agencies in order to fully understand and navigate issues around supporting discharges from beds to home.

Promoting reablement where beneficial for both post and pre discharge. Responsibility to work alongside systemwide operational digital colleagues to develop systemwide dashboard. Management of performance in line with key performance indicators.

Complete and maintain accurate records and databases (electronic and paper) using the appropriate systems in place ensuring information governance (IG), general data protection regulation (GDPR) guidance is followed, and confidentiality maintained at all times.

Participate and contribute to MDT meetings, raising challenges and solutions to complex situations. Contributing to service development.

Maintain own continuing professional development (CPD) and act in accordance with the relevant professional body, Code of ethics and professional conduct.

Collate and coordinate feedback to system providers around challenges and ensuring timely escalation and dissemination. Problem solving and dynamic approach to discharge facilitation. Participate in on call rota. To undertake additional duties as deemed appropriate by line manager.
