We are looking to recruit a Team Leader to join our fast-paced, multi-disciplinary Reablement Team working within the London Borough of Ealing.

The role is 35 hours Monday to Friday with duty week and weekend cover as required. The hours for this are 7am to 9pm.

Have you got the ability to promote independence, supporting individuals to achieve their goals and to remain as independent as possible within their own home, as well as supervise care staff?

You will need to have:

Three years' experience of managing a similar service within local government and /or health sector.

A nationally recognised qualification in management e.g CMS, DMS or NVQ Level 4 or the willingness to undertake NVQ
Substantial experience of working with older people, people with disability, mental health including Dementia and other vulnerable groups of service users
Ability to undertake assessments, set goals, monitor progress and work as part of a multidisciplinary team

- the ability to organise / deliver personal care services to customers in either a paid or unpaid capacity, including experience of working in a paid supervisory capacity.

Good IT knowledge including experience of staff rostering systems, excel, word and the ability to compile statistics for service as required.
