We now have an exciting opportunity for a Dementia Support Worker to join a team of committed professionals in our Community Support Services Team.


This is an exciting role that will provide information, advice and practical and emotional support to older persons and those living with dementia.

The role will work to facilitate choice, independence and person-centred support, including where appropriate informed decision-making and valid consent as per the Mental Capacity Act

You will respond to referrals, assessment of needs and work in partnership with individuals, their families, and carers, to build independence and personal resilience, enabling self-management of their health and well-being


You will work within the Community Support Services team to develop new, innovative and more effective ways of working, including development of a dynamic range of activities, that builds independence, personal resilience, enabling self-management.

Willingness to learn new models of support to help people live as independently as possible with Dementia

You will need to be flexible and able to adapt to the pace of a fast-changing environment, with great communication skills

You will need to be have good record keeping skills to ensure that electronic client records, activity, progress towards outcomes, and other monitoring and information is completed accurately and in a timely manner

Safeguarding, monitoring and reporting are essential to this role.
