This exciting opportunity will include working within a speciality team to manage patient prescribed Domiciliary Oxygen, to be responsible for the day-to-day co-ordination of the service and data management of the St Helens Oxygen Service.
The post holder will be proficient and competent to work across professional disciplines, referring clients for assessment and treatment or to other agencies as required.
To monitor and instigate reviews which will be both carried out virtually in the office and in the patient's homes to ensure all patients receive appointments in a timely fashion as required.
The post holder must have key computer skills including the ability to use excel spreadsheets to interrogate oxygen data, raising invoice queries to generate refunds and to plan nursing colleagues' workload.
The care coordinator will assist in the identification of health needs and participate in the delivery of St Helens Home Oxygen Service (HOS-AR) under the supervision of a Registered Practitioner.
The post holder will continue to develop and undertake a range of delegated nursing tasks across primary and secondary care and will report to the Specialist Nurse.
Will be the first point of contact for all Domiciliary Oxygen Service queries from patients, Clinical Commissioning Groups, Oxygen providers and staff within Primary and Secondary care
Responsible for supporting the operational and administrative functioning of the Home Oxygen Service under the support of the Oxygen Team Leader
The post holder will be responsible for referral management and will be able to demonstrate their professional skills and competence in leadership, planning and coordination.
Accountable for reviewing whether a referral to the service meets the established referral criteria and is compliant with referral guidelines.
Responsible for ensuring appropriate information is readily available to members within the Multidisciplinary Team (MDT) and that this information is regularly updated.
Heavily support the discharge process of Oxygen patients by working collaboratively with Respiratory Nurses across primary and secondary care, oxygen providers and the Medicines Management Team.

The Secretary of State has approved the partnership between St Helens & Knowsley and Southport & Ormskirk Hospitals.

Key Duties Continued
Liaise closely with MDT team members involved with the Oxygen Service and related departments and will provide admin support for the Oxygen Service.
Liaise with clinicians, Oxygen Nurse Team Leader, and Respiratory Nurse Specialist regarding coordinating oxygen assessments across primary and secondary care and clinic administration.
Work collaboratively with health professionals and other agencies to ensure patient needs are met and devise an individual plan for discharge planning -from admission to discharge.
This will also involve working with other members of the MDT group i.e. General Practitioners, Community Matrons, PalliativeMore jobs from Mersey and West Lancashire Teaching Hospitals NHS Trust
