A fantastic opportunity has arisen and North Preston Medical Practice are looking to recruit an Advanced Clinical Practitioner (ACP) to complement our team.

We are looking for an ACP to consult autonomously within their professional scope of practice providing face to face and telephone consultations.

The ACP will be enthusiastic, flexible, reliable with Independent Prescribing status.

The ACP will work within their sphere of competence and will provide care for our patients. You will demonstrate safe, competent clinical decision making. You will work collaboratively with the practice team to meet the needs of our patients.

The following are the core responsibilities of the Advanced Clinical Practitioner.

  • There may be, on occasion, a requirement to carry out other tasks; this will be dependent upon factors such as workload and staffing levels:_
  • Practice in compliance with their respective code of professional conduct and within their scope of practice, being responsible and accountable for their decisions, actions and omissions at this level of practice
  • Demonstrate a critical understanding of their broadened level of responsibility and autonomy and the limits of own competence and professional scope of practice, including when working with complexity, risk, uncertainty and incomplete information
  • Act on professional judgement about when to seek help, demonstrating critical reflection on own practice, selfawareness, emotional intelligence, and openness to change
  • Work in partnership with individuals, families and carers, using a range of assessment methods as appropriate
  • Demonstrate effective communication skills, supporting people in making decisions, planning care or seeking to make a positive change
  • Use expertise and decisionmaking skills to inform clinical reasoning
  • Initiate, evaluate and modify a range of interventions which may include prescribing medicines, therapies, lifestyle advice and care
  • Exercise professional judgement to manage risk
  • Implement and evaluate individual specialised treatment plans for chronic disease patients
  • Identify, manage and support patients at risk of developing longterm conditions, preventing adverse effects to the patient's health
  • Be a professional role model for the organisation
  • Actively engage in peer review to inform own and other's practice, formulating and implementing strategies to act on learning and make improvement.
  • Actively seek feedback and involvement from individuals, families, carers, communities and colleagues in the coproduction of service improvements
  • Critically assess and address own learning needs, negotiating a personal development plan that reflects the breadth of ongoing professional development across the four pillars of advanced clinical practice (clinical practice, leadership and management, education and research).
  • Provide chronic disease clinics, delivering patient care as necessary, referring patients to secondary/specialist care as required
  • Maintain accurate clinical records in conjunction with extant legislation and to ensure SNOMED CT codes are used effectively
  • Critically appraise and synthesise the outcome of relevant research, evaluation and audit, using the results to underpin own practice and to inform that of others
  • Develop and implement robust governance systems and contribute to practice targets (QOF, etc.), complying with local and regional guidance
  • Chaperone patients where necessary
  • Support the team in dealing with clinical emergencies
  • Support the team with all safeguarding matters in accordance with local and national policies
  • Understand practice and local policies for substance abuse and addictive behaviour, referring patients appropriately
  • Deliver opportunistic health promotion where appropriate
  • Conduct comprehensive patient assessments, including medical history, physical examinations, and diagnostic tests
  • Develop and implement individualized treatment plans in collaboration with the healthcare team
  • Monitor patient progress and make necessary adjustments to treatment plans
  • Educate patients and their families on disease management, prevention strategies, and healthy lifestyle choices
  • Collaborate with other healthcare professionals to ensure coordinated and efficient care deli
