CASE MANAGER ASSISTANT Salary:
Band 3

Full Time: 37 hours per week Would you like to join our Case Management and Discharge Hub Team? Do you have previous experience working in a healthcare setting and have an understanding of discharge or do you have experience in administration? We are looking for a team member to join us to support the discharge and flow of patients within the Royal Surrey NHS FT and community hospital sites.

The Case Management Service works alongside the Multi-disciplinary Team to support the patients pathway through the hospital.
