Description

The successful post holder will work under the direction of a clinical lead, GP/Qualified Nurse and assist in the implementation of patient care both direct and non-direct.


Care coordinators play an important role within a Primary Care Network to proactively identify and work with people, including the frail/elderly and those with long-term conditions, to provide coordination and navigation of care and support across health and care services.

The successful post holder will be based at Seaton Delaval NHS manufacturing hub and will work for Rothbury Medical Practice and Ponteland Medical Group

  • Use judgment to translate cases where the information included is not readily codable.
  • Care coordinators review patients' needs and help them access the services and support they require to understand and manage their own health and wellbeing, referring to social prescribing link workers, health and wellbeing coaches, and other professionals where appropriate.
  • Regularly liaise with the range of multidisciplinary professionals and colleagues involved in the person's care, facilitating a coordinated approach and ensuring everyone is kept up to date so that any issues or concerns can be appropriately addressed and supported;
  • To ensure patient confidentiality is maintained in accordance with Trust policy


Northumbria Primary Care Ltd (NPC) is a pioneering partnership with local GPs and now manages several GP practices with around 50,000 registered patients in Northumberland and North Tyneside.


Whilst appointees will have a base practice within NPC, they may be asked to work at other practices across our group.

We do however limit this as much as possible as we appreciate continuity is vital to effective primary care
