Job overview:


This role is an exciting opportunity for a highly motivated and experienced clinician with a proven track record in education, leadership and care of patients with complex long term conditions.


As part of the Ageing Well Programme in Cornwall, the focus for the successful applicant will be to provide continuing educational support and clinical coaching to existing community clinicians.

Reflecting the growing demands and clinical complexity of our population in Cornwall, we have committed to developing the skills and versatility of our workforce.

This will require the post holder to deliver clinical education, leadership and supervision to our growing and developing workforce, this will include supporting the community transformation programme.


As a highly visible leader, with exceptional communication skills you will be based within North and East Integrated Care area (ICA) reporting managerially to the Head of integrated Professions and supervised by the Clinical lead for frailty and ageing well.

You will be an integral role model delivering 1:1 clinical coaching within community teams, supporting the students participating in the Ageing well education programme.

Additionally you will be shadowing other community clinicians undertaking patient care, providing support and education at the bed side, with a focus on optimising the management of frailty and long term conditions and promoting an ethos of ageing well.


Main duties of the job:


You will be required to participate in the leadership and delivery of formal class room based teaching, both within North and East ICA or as part of a countywide delivery of education.


As a member of the ageing well development team which consists of the clinical lead, programme support manager and 3 clinical facilitators you will help to inform and design future training to meet the complex needs of our growing older population.

You will also be pivotal in supporting the community transformation programme.


The focus of your role will be in ensuring clinicians are confident to undertake comprehensive assessments of people with complex long term conditions and frailty ensuring that high quality, safe and effective care is provided.
