PCN Operations Lead


Hours of Work: 16 hours - Tuesday and Thursday (OFFICE)


Salary:
£35,000 - £40,000 PRO RATA, dependent on experience


Reports to:
PCN Clinical Director, PCN Board


Office Base:

Overall Job Purpose


Responsible for managing the implementation and delivery of the PCN DES Service requirements across the PCN, supporting the achievement of the Impact & Investment Fund (IIF) targets across PCN practices, as well as operational management of other identified projects and services the PCN are required to deliver annually.


The role will also involve liaising with PCN Practices and other stakeholders to promote engagement and understanding of the PCN specifications and requirements for implementation.


The post holder will work with the PCN Management Team (including the PCN Clinical Director, and PCN Business Manager) to progress new and innovative service developments responding to emerging knowledge and techniques, reporting to the Clinical Director.


Key duties and responsibilities

Operations/Service Delivery

  • To provide oversight and coordinate delivery of the PCN DES specifications and IIF targets as relevant. To liaise with PCN leads in relevant areas.
  • Ensure all relevant national strategies, policies, targets, and frameworks are fully implemented as required.
  • Ensure a whole system personalised care approach across the PCN that enables a variety of services across the health, social care, public health, and community spectrum to be integrated around the individual in order to deliver better outcomes and experiences.
  • Deliver on the PCN guidance documents associated with the PCN DES (ie enhanced health in care homes, personalised care)
  • To drive issues through to resolution, acting as a point of expertise
  • Work with the PCN Clinical Director, PCN Business Manager and board for strategic and operational planning processes to support PCN objectives as above
  • To implement networkbased projects and services, alongside PCN management team.

Strategic Management and Leadership

  • Keep abreast of current affairs and identify potential threats and opportunities.
  • Contribute to PCN strategy.
  • Monitor and evaluate performance of the PCN service requirements.
  • Develop and maintain effective communication within the PCN and the PCN member practices and all relevant stakeholders (ICB, Federation, Patients etc)
  • Develop and seek out opportunities to promote the PCN and the services it offers.

Relationship Management:


  • To represent the network at relevant external meetings
  • To foster a culture of collaboration and promote learning to drive continuous improvement.
  • To work as a flexible member of the PCN, providing support to other team members when necessary
  • To take an active role in the development and embedding of the PCN's culture, values and reputation
  • To represent the network at external meetings
  • Build relationships and facilitate collaboration between practices within the network to support the delivery of care to patients (e.g., delivery of enhanced access or other hubtype working arrangements)


The post holder may be required to undertake other duties should the post require it and will be required to travel to various locations on occasions to fulfil the requirements of the role.

Qualifications

ESSENTIAL

  • Good standard of education with excellent literacy and numeracy skills
  • Leadership and/or management qualification (or equivalent experience)

DESIRABLE

  • Educated to degree or masters level in healthcare or business
  • AMSPAR qualification level 5 or equivalent

Experience

ESSENTIAL

  • Experience of working in a healthcare setting
  • Experience of Operational Management and Service Delivery
  • Experience of working with the MDTs
  • Experience of developing new services
  • Experience of leading on service improvements, and making tangible positive transformation in an organization
Track record of embedding quality assurance systems


DESIRABLE

  • Experience of working within a Primary Care Network, or GP Practice
