South Tees Hospitals NHS Foundation Trust is one of the largest teaching hospital trusts in the country, close to the North Yorkshire Moors and within easy reach of Newcastle and Leeds.

It comprises two acute hospitals, The James Cook University Hospital and Friarage Hospital Northallerton as well as established community service teams across Middlesbrough, Redcar & Cleveland, Hambleton and Richmondshire localities.


This exciting new role has been developed to support the developed 'home first' multidisciplinary team working across the South Tees locality.


This is an opportunity for those who are interested in working in the local community and being part of an exciting model of working in the community localities of Middlesbrough, Redcar & Cleveland.

The service aims to create effective, integrated community services that enable patients to be cared for as close to home as possible, support them in achieving their optimum health and function, and reduce emergency admissions and lengths of stay in hospital.

The post holder will work as part of the UCR/Virtual Ward/Home first team ensuring the safe discharge of patients, providing follow up re-ablement and care for all patients.


You will need to be able to work on your own; as part of a team and be involved with individuals with a range of medical, social and mental health conditions, therefore knowledge and experience of this is needed.

You will hold a designated clinical caseload within agreed protocols and carry out therapeutic interventions and care programmes under the supervision of qualified therapists and nurses.


Excellent communication skills, organisational skills, enthusiasm, ability to travel around the locality, commitment and ability to work with a wide range of agencies are also vital in this post.


This is an opportunity for anyone with previous experience working within nursing, therapy or in care settings to help bridge the gap between hospital and home.

The Home First Team will identify patients on the wards at The James Cook University Hospital that can be assessed in their own home to facilitate early discharge.

Then as a senior integrated support assistant you will provide both rehab and care following a person centred plan within the patients' own home post discharge.


South Tees Hospitals NHS Foundation Trust offers leadership and improvement training to all new staff to the Trust; this training programme has been designed to support our leaders in developing their understanding of leadership and management skills.

You will be expected to attend the New and Aspiring leaders programme, and the Foundation Quality Improvement Programme that the Trust delivers.

After you complete this one and a half day of training you will be able to explore further leadership and improvement training opportunities, we offer further in-house courses and bitesize programmes and leadership apprenticeships at level 3, 5 and 7.
