Reporting to:

Head of Nursing/Head of Rehabilitation


Location:

Office hours at home then delivering in the community (ideally located in the Birmingham / West Midlands area to cover the M6 corridor) must be willing to travel and occasional overnight stays.


Job Purpose:


The overall aim of the role is to achieve the highest possible standard of care in a professional manner, through providing direct and indirect care to, including but not limited to, people we support experiencing mental health problems, behaviours perceived as challenging and acquired brain injuries.

This will also include being part of the assessment process to help people move out of hospital and into their own home.


Essential:


  • Minimum 2 years postgraduate experience.
  • Experience working with people who may present with behaviours perceived as challenging, personality disorder, or mental illness.
  • Excellent communication skills
  • Excellent IT skills
  • Knowledge and ability to undertake robust holistic assessments as required.
  • Have the ability to support people in crisis and work collaboratively with the MDT to manage this.
  • Experience working with a multidisciplinary team along with other professions such as OT and psychology, care coordinators and health commissioners.
  • Knowledge of Mental Health Act and Mental Capacity Act. (This includes DOLS, CTO AND MOJ)

Main Responsibilities:


  • Caseload management of people we support with complex needs.
  • Building relationships with people we support to encourage trust, while listening and interpreting their needs and concerns.
  • Working within a CPA framework. (CTO, MHA, MLA, MOJ).
  • Manage your time efficiently including diarising appointments and logistics of getting to and from your appointments.
  • Assessments for new business.
  • Ensure uptodate risk assessments and recovery plans/PBS are in place.
  • Report writing.
  • Liaise with relatives of the person we support.
  • To communicate within the wider GHC team ensuring best level of care for the person and support for the staff.
  • To work collaboratively with staff teams to motivate the person we support to ensure they have a full weekly activity plan.
  • Ability to use initiative and problemsolving skills to assist with crisis intervention.
  • To maintain CPD.
  • To access and offer supervisions as appropriate.
  • On Call every 6 weeks as per rota.
  • RMN/OT
  • Current MNC/HCPC registration and educated to degree level.
  • Evidence of updating practice and continued professional development.
  • Willing to undertake education/courses relevant to area of practice and service development.

A minimum of 2 years post registration experience in the UK
:


  • Ability to assess, plan, implement and evaluate care and ability to develop a therapeutic relationship.
  • Experience of managing a caseload.
  • A range of experience within hospital and community settings.
  • Understanding of safe and effective alternatives to hospital care.
  • Understanding of the Mental Health Act / Mental capacity act.
  • Highly developed risk assessment and management skills.
  • Highly developed communication skills.
  • Ability to work within a multidisciplinary team.
  • Ability to engage people we support in their care.
  • Ability to present information both written and oral and communicate this effectively to a range of audience.
  • Excellence in report writing and IT skills.
