Job summary


An exciting opportunity in Hastings primary care to join a large and fast-growing PCN as part of the Therapeutic Team (PCN-based Social Prescribing, Occupational Therapy and Health Coaching), supporting these services in terms of all administration, referral coordination, liaising with practices, diary management and research.

The role offers the opportunity to join the NHS pension and working from home flexibility.


Main duties of the job


The post holder will support the therapeutic team by providing extra time, capacity, and expertise to support patients with a variety of complex conditions.

They will help manage a caseload of patients for each therapeutic service including managing referrals, making initial contact with patients and providing updates, scheduling appointments, entering data into clinical systems and research.


As part of the role they will work closely with practice based Care-Coordinators on a variety of PCN projects, as well as PCN Social Prescribing and Occupational Therapy and Health Coaching teams.


About us
Hastings and St. Leonards Primary Care Network is a large PCN comprising 9 GP practices that serve a population of 100,000 patients.

The network team works closely with each of the constituent practices to provide collaborative services, resolve shared problems and explore shared opportunities.

The PCN team includes a dedicated infrastructure of Clinical Directors, Practice Representatives, Operational and Financial Management and Clinical and Non-Clinical Service teams including Covid Vaccination, Health Coaching, Clinical Pharmacy, First Contact Physiotherapy, Social Prescribing and Occupational Therapy.


Job responsibilities
The following is a non-exhaustive list of core responsibilities:

  • Incoming referral management
  • Corresponding with GPs, Nurses and Care Coordinators from local surgeries regarding referrals
  • Contacting patients and arranging appointments
  • Contact patients on the waiting list with updates on waiting times
  • Administration of incoming referral system
  • Regularly manage Occupational Therapist appointment diary, as required
  • Ordering equipment
  • Managing database of patients and sessions
  • Independently carry out agreed treatment plan completed by the OT with patients, feeding back to the OT
  • Exploring available information for the patient, such as specific exercises, relevant voluntary groups, liaising with the OT as required
  • Keep OT spreadsheet up to date and provide regular reports for the wider PCN team and the surgeries, as required
  • Assisting with group work and group session planning
  • Updating information on other services as required
  • Signposting patients to other services.
  • Creating discharge summaries and liaising with referrers
  • Onward referral management
  • Monitor follow ups required for patients
  • Support OT in regularly producing OT newsletter to update the PCN and surgeries, including referrals, work being undertaken and feedback from patients
  • Managing patient feedback

Person Specification

Essential Experience

  • Knowledge and experience of general administration processes
  • Good organisational skills
  • Confident and effective communication skills, both verbally and in writing
  • Good interpersonal skills
  • The ability to be patient, caring, calm and empathetic
  • The ability to keep calm under pressure
  • The ability to work as a member of a team, showing ability to work flexibly, as required
  • The ability to work under direction, meeting deadlines, as well as being able to work alone, as required
  • The ability to respond to appropriate requests for assistance, as well as being proactive to complete a task, as required
  • Manage and prioritise multiple tasks

  • IT literacy including proficiency in word processing, excel and data entry

Desirable Experience

  • Previous experience of working in General Practice
  • Competent in using clinical systems, such as EMIS
