JOB TITLE:
Medical Records Summariser /Clinical coder/ Typist





REPORTS TO:
Practice Manager





HOURS:
15-25 hours per week (2 posts)


Job Summary:

To be responsible for the provision of an efficient, comprehensive secretarial and administrative support service to the Practice.


The main responsibilities of the post-holder will be to enter Read Code relevant information from patients' clinical records into the practice computer system, processing of patient referrals, and summarising patient notes.


Main Duties/Key Tasks:


  • Audio and Copy typing of hospital referrals and letters
  • Processing referrals onto Electronic Referral System following practice protocols and procedures.
  • Liaising with external agencies such as hospitals and community services, ensuring referrals are processed efficiently
  • Reviewing medical records and producing an accurate summary of the patient's medical history.
  • Entering correct Read Code information from paper records into the practice clinical system (SystmOne)
  • Scanning of patient related documentation and attaching scanned documents to patient's healthcare records
  • Coding clinical information from letters, discharge summaries, notes etc.
  • Prepare medical reports and generate letters of correspondence relating to the management of patient care.
  • Submit & Process Advice & Guidance requests.
  • Monitor and track referrals ensuring all criteria is met.
  • Monitoring progress of notes summarising against practice targets
  • Maintaining awareness of clinical code changes and introduction of new codes
  • Prioritise and Complete tasks received in a timely manner
  • Perform any reasonable administrative tasks as requested

Skills

  • Computer literate and proficient in using Microsoft Office Package
  • Have a good understanding of Medical Terminology

  • Using appropriate infection control procedures, maintaining work areas in a tidy and safe way and free from hazards
  • Reporting potential risks identified.
