An exciting opportunity has arisen to join the Volunteer Hub at City Health Care Partnership CIC.

We are looking for a motivated and enthusiastic individual to join our team in the role of Health Tutor for our EMPOWER programme, the Chronic Disease Self-Management Programme.


The programme offers free self-management courses to those who suffer from long-term health conditions, As a Health Tutor you will be required to work with the local community to facilitate groups.

You will possess excellent communication skills in order to motivate and support individuals in making positive changes to their lifestyles.

The post holder will join the Volunteer Hub team with specific responsibility for delivering the EMPOWER Programme, the Chronic Disease Self-Management Programme.

The programme offers free self-management courses to those who suffer from long-term health conditions and is run by volunteer tutors who also suffer from long-term conditions.


As a Health Tutor you will be required to work with the local community and deliver the programme to groups.

You will possess excellent communication skills in order to motivate and support them in making positive changes to their lifestyles.

The post holder will deliver the EMPOWER programme in a group setting in a variety of settings e.g. community venues and health care buildings.
