Qualis Solutions have an exciting opportunity for someone to work with a good rated CQC Home care provider who are looking for a Care co-ordinator in Hertford, Hertfordshire.

We are looking for somebody who is motivated, proactive and who is unafraid to roll up their sleeves to get the job done.

An excellent opportunity has now arisen for a
Care Co-ordinator to join our home care team in Hertford, on a permanent full-time basis.

We are a provider of high-quality home care services.

We are looking for a kind, compassionate, and highly motivated person to join our Hertford branch and support our staff caring for individuals, in their own homes.

Previous experience as a
Care Co-ordinator is essential. Experience of Rota Management and Electronic Call Monitoring Systems is essential, as you will be required to undertake Co-ordinating. NVQ 3 (or above), in Health and Social Care, essential.

Requirements:

• To be able to undertake Care Coordinating.
• To be able to undertake initial care needs assessments, reviews, and writing of all care documents, including care plans and risk assessments.
• To have empathy and understanding
• To be reliable and willing to work for the best interests of the people we care for
• To be able to communicate effectively and professionally.
• To be a driver (with your own transport)
