An exciting opportunity has arisen to recruit a motivated and enthusiastic Band 3 Healthcare Assistant on a fixed term basis, to work with the evolving Community Team, as part of the developing Integrated Care Community (ICC) you will be based within Eden ICC, Lonsdale Unit.

ICCs aim to address the challenges of an aging population and increasing demand on health and care services by providing more out of hospital care and empowering people to take control of their health and wellbeing.

This will require new ways of working to focus on preventing ill health and ensure services are sustainable for the future.

These new ways of working will follow the same approach across all eight ICCs with flexibility to tailor services to the needs of the local people.


This role requires you to demonstrate the skills and knowledge to support the provision of high-quality care, to support patients with complex needs within the community setting.


This is an excellent opportunity for someone who is looking to develop their clinical skills and expertise in caring for patients, working closely with health and social care colleagues to prevent hospital admission, facilitate hospital discharge and promote independence through reablement and management of long-term conditions.

Daily working hours are between 8:00 and 23:00.

As a Health Care Assistant in a multi-disciplinary team, work with other health and social care professionals to develop the ethos of multi-disciplinary, integrated care and seamless services ensuring that patient care is delivered to the highest possible standard.


As a Health Care Assistant, working under the direction of a registered health care professional, undertake a range of delegated patient nursing and care duties including clinical observations and recording patient information.

This will include supporting patients with personal hygiene and activities of daily living.


Working largely alone and unsupervised, assess patients, plan and implement care in the community, provide advice and education to patients, carers and colleagues, maintain associated records.

Carry out nursing procedures.

Carry out delegated management tasks including line management of other team members.

Flexible working is required to meet the needs of the patients group.

The Care Group is commissioned to provide 24/7 nursing services.
