In order to meet the growing demand for supporting people in North Lincolnshire, Home first community support are looking to recruit x 2 Community Rehabilitation Officers.


North Lincolnshire Home First Community support is a registered Rehabilitation and Reablement service that provides time limited intervention and therapies to people within their own homes.

Our aim is to ensure that a person can remain at home and feel confident and is safe to live as independently as possible following a period of illness or injury.

We work in partnership with other social care and health professionals to prevent care needs from escalating and also work to prevent avoidable admissions and facilitate appropriate early discharge from hospital via working with the integrated discharge team.

Our Rehabilitation service 'helps people do things for themselves' rather than 'doing it for them' through short term intensive support programmes that enables people to optimise their daily living skills and well being.

The role will involve working with individuals and their circle of support to regain their confidence and move forward with their life - based on their abilities and strengths and those of their family and community.


Home first Community support is also the Local Authorities crisis out of hours provider ensuring the safety and well being of vulnerable adults with social care needs across the whole of North Lincolnshire 365/24/7.

You will therefore under the direction of the senior on duty be required to be responsive to such crisis intervention and work with the service user and their circle of support to make safe and enable individuals to subsequently regain control of their lives.


As a rehabilitation officer you will need to be solution focussed, have the ability to motivate others, have excellent decision making and interpersonal skills and be able to work autonomously in complex situations to determine need and facilitate progress through the service.


You will also be responsible for providing appropriate day to day supervision of a designated number of Community Reablement Assistants, to include supervision, observations and competency assessments.

The ability to work flexibly including weekends and unsocial hours, including bank holidays, and be able to travel around North Lincolnshire is essential


In addition following training you will be required to undertake prescribing of low level equipment across the service and act as a key trainer, providing moving and handling training and guidance to staff.
