An exciting opportunity has arisen to join the East Riding of Yorkshire Council, whilst we embark on our Discharge to Assess programme.

You will have the opportunity to support shaping the future of Hospital Social Work, supporting discharges into the community from Hull Royal Infirmary and Castle Hill Hospital sites.

Due to the increased demand and development of the team we have a number of permanent posts to support with the safe and timely discharges from the acute hospitals and reduce delays by proactively facilitating discharges.

The positions will be based within both Hull Royal Infirmary and Castle Hill Hospital with a blended home and office working option.


Currently the team cover business hours of 8am - 5pm on a rolling 4-week rota with a current rota of working 1 in 4 weekends.

The service covers 364 days a year with the exception of Christmas day.

The role of a care coordinator is an exciting opportunity to develop skills in assessing, reviewing and care planning for clients in need of support.

The service will support the development of the required skills and has amazing opportunities to develop your career further.

You must have excellent communication skills, the resilience to work under pressure and be flexible in your approach.
