Our company provides housing and support to individuals with a learning disability, autism, or mental health problems in High Wycombe.

We support people in registered care settings, supported living services within people's own homes.

The company supports individuals with ASD and Mental health, with a person-centered approach to live and access the community around them.


Job scope:

An opportunity has arisen for a Team Leader to join our growing company in High Wycombe. We are seeking a Team Leader who has experience working with adults with physical and learning disabilities.


We are looking for someone who has have experience providing support to staff, and also providing emotional, educational, and physical support to service users.

You must be determined, empathetic and committed to making a difference to our service users' lives. The role is to work in collaboration with the Deputy Manager and Service Manager within our Residential Care Service.


Job Summary:


  • To ensure that the home or service provides and maintains the highest levels of support and services in accordance with the company's Mission Statement, Statement of Purpose and policies and procedures.
  • To monitor and manage highquality personcentered support in line with the values and philosophy of the organization.
  • To ensure that the service provision is underpinned by a commitment to the company's Equal Opportunity policy and procedures.
  • To manage and monitor the daily running of the home or service and be responsible for allocating duties on shift.
  • Ensure that the Support Workers carry out their duties and responsibilities, with guidance and support and provide mentoring to support staff where necessary.
  • To ensure that service users' needs are fully met and that they are fully involved in all aspects of their lives and to encourage individuals to be as independent as possible.
  • To utilize assurance reviews to ensure that all required care standards and legal requirements are met. When Internal audits identify correction, these are actioned and recorded within the agreed time scale.
  • To monitor and manage all administration requirements within the home/service, under the supervision of the manager.
  • To work with the multidisciplinary team as required and adhere to the principles if the Mental Capacity Act, to ensure that best interests are identified for each service user and plans are established to meet them.
  • To be responsible for all financial resources allocated to the Team Leader.
  • To manage support staff and be involved in recruitment, promotions and disciplinary procedures when required.
  • To liaise with external professionals and bodies including CQC, Social services and health professionals.
  • To undertake annual appraisals and support staff supervisions in accordance with company policy and CQC requirements.
  • To be involved in the assessment process with prospective service users and assist with all transactions for the home or service.
  • To work in partnership with the Manager to achieve the best outcomes for the home or service.

Duties and Responsibilities:


Service Users:


  • To actively promote a personcentered approach to support service users. To ensure that the individual needs of all service users are met daily by the team and that all appropriate meetings are held and recorded as required.
  • To ensure that all accurate and current records are maintained for each service user and that effective contact is maintained with other professionals involved.
  • To be responsible for monitoring and attending all service users' reviews and monitoring any follow up action required.
  • To liaise effectively with all family members and ensure that they're informed of relevant meetings, reviews, or company events.
  • To work in partnership with involved professionals and promote their work within the team for the benefit of service users.
  • To ensure that service users and staff are enabled to make formal complaints in accordance with the company's policies.
  • To ensure that service users are involved in decisions that may affect them.
  • As a Team Leader; to personally model and promote a high standard of personal care for each person including intimate care, hygiene, and physical wellbeing.
  • To respond appropriately to medical or health concerns and administer medication in accordance with the company's policies and procedures.
  • To act as a key worker for named individuals; to oversee the coordination of their care plans and risk assessments.

Staff Management:


  • To be responsible for allocation of roles and tasks during a shift and to always maintain its effective operation.
