As part of the Community Transformation, we are seeking to recruit a Clinical Specialist to work in the newly developed Community Interface Team in Hartlepool.

The role is looking towards enhancing and clinically leading the team within the new community interface team - working closely with all NHS and other agencies to develop a collaborative approach to patient care.

The new ways of working will work closely with the service users and their families to ensure they receive the right care from the right service in a timely manner.
