Description

Eden Brown are looking for Adult Social Workers for the Home First Team in The Wirral.

We are looking for social workers to support the Discharge to Assess base at Clatterbridge in the Wirral.

It's an inpatient unit compromising of 3 wards where people are discharged from hospital for further rehab and social care assessment.

Target length of stay 21 days so rehab and assessments for longer term plan to discharge home or to 24 hour care dependent on needs.

Anyone with Adult Social Care experience will be considered.
