Description

We are looking for a Clinical Co-Ordinator for our Children's Rapid Response and Palliative Care Teams to provide professional and clinical leadership working alongside the Team Leaders and Clinical Leads.


You will be overseeing the effective and efficient use of skills and resources to ensure the team and the service is organised to meet the needs of the children, young people and their families.

The aim of the service is to offer a high standard of care to children, young people and their families, reducing and preventing unnecessary hospital attendances and admissions when it is clinically safe to do so for children and young people already known to the Children's Community Nursing or Palliative Care Teams
