Are you a band 3 therapy assistant or rehab assistant looking to take a step up? Are you looking for a challenge and a change? We have an exciting opportunity within the Uttlesford Community therapy service.

We are seeking to recruit a dynamic and forward thinking rehab worker to support out integrated community team working with patients to achieve their goals and regain independence.


The role of the Therapy Assistant Practitioner is to support the Uttlesford integrated community care team to provide physiotherapy and occupational therapy support.

Band 4s work closely with all users of our services and their families, friends and carers, who all play an invaluable contribution in how our users experience our services.

Band 4s are expected to be kind and responsive but professional and informative and contribute to the quality of the services provided across the health economy

  • To work as part of the multidisciplinary team to prevent admission, support early discharge an
  • To deliver specific therapeutic input as part of goal directed treatment and rehabilitation programm
  • To assist in the delivery of therapy interventions and reviews as appropriate
  • To feedback any outcomes/concerns/progression/deterioration with regard to the achievement of the set goals.


The Assistant Practitioner does not require daily supervision and should be able to take responsibility for their work within pathways of care and that of others within a broad framework laid down by and as directed by the supervisor of practice.

  • Follow up Assessment, planning, implementation and evaluation of delegated caseload and escalating changes to the Registered Practitioner
  • To support in the supervision of Junior Health Care Support Workers and Trainees
  • To actively work as a member of the multiprofessional team to provide high quality care to patients/clients
  • To undertake, and report back on delegated clinical activities that have previously been signed competent
  • To undertake clinical interventions utilising a variety of modes, including planning and leading group and individual therapy and issuing appropriate equipment as part of packages of prescribed care after achieving appropriate competencies.
  • Supporting community teams with accessing patient info when out on visits
  • To maintain patient records using appropriate documentation for intervention carried out.
  • To enable and empower patients to develop self and environmental management skills.
  • To promote and facilitate access to community resources.
  • Enable individuals to sustain and improve their overall health and wellbeing.
  • Identify, act and minimise risk to patients and clients.
  • Exchange information with patients/clients requiring support, tact and reassurance.
  • Form professional relationships with staff/patients/clients and communicate and cooperates with them in a way that respects their views, autonomy and culture.
  • Constructively manage barriers to effective communication and works cooperatively with patients and team members.
  • Work without direct supervision of the registered practitioner in the implementation of programmes/reviews/triage tasks appropriate to the community and evaluate
