Please see full details in job description Practice autonomously as an ACP using collaborative and independent judgement to assess, investigate, diagnose, plan, implement, manage, and evaluate clinical nursing and medical care in the management of patients.

You will be responsible for the initial treatment of patients presenting with undifferentiated or undiagnosed conditions and make professional autonomous decisions regarding treatment within your scope of practice Prescribe medicines independently and review medication for therapeutic effectiveness, appropriate to patient needs and in accordance with evidence-based practice, national and practice protocols, and within scope of practice to optimise treatment.

Liaise with other members of the multidisciplinary team to plan the safe discharge of patients when required Liaise with nursing staff, Consultants and junior doctors on an on-going basis, informing them of patients management plans as appropriate Plan and co-ordinate the patients assessment, treatment and discharge/admission Participate in audit, clinical governance and other clinical meetings where the outcomes and recommendations have the potential to improve the quality of care; give case presentations as required.

Contribute to the development and implementation of clinical policies, procedures and guidelines Promote the delivery of evidence-based care to all patients, recognising the limitations of the evidence allowing autonomous decisions to be made Teach and support medical staff, student and newly qualified/less experienced ANPs/ENP, junior and senior nurses, student nurses/midwives, paramedic trainees, health care assistants and other staff, as appropriate in the clinical setting.
