Purpose
We are looking to recruit an experienced Clinical Coder & Summariser to complement our existing team. We offer competitive rates of pay and NHS Pension. We are looking for someone to work across 3 days - Monday, Thursday and Friday.
This role would suit someone who is looking for an opportunity to develop their existing coding & summarising skills as the main responsibilities of the post-holder will be to triage both incoming hardcopy and electronic post, work-flowing to the GPs as appropriate.
They will need to code relevant information from patients clinical records into the practice EMIS web system, working strictly in accordance with practice protocols, with the highest levels of accuracy and attention to detail.
We are looking for someone who has excellent verbal and written communication skills, and who is proficient in the use of Microsoft Office Suite.
**Duties:

- Opening and sorting the incoming post
- To triage incoming electronic and hardcopy post and workflow to the GPs as appropriate
- Culling and sorting patient records and letters in preparation for summarising
- Reviewing medical records and producing an accurate summary of the patients medical history
- Summarising incoming electronic and hard copy post according to agreed protocols
- Read coding medical information into the practice clinical system
- Ensure summarising of patient notes is accurate and succinct.
- Correcting any anomalies in patient notes as required
- Restructuring existing computerised notes as necessary
- Dealing with queries in relation to coding
- Clinical coding of relevant information within documents prior to clinical review (Docman triage)
- Daily administration and coding of incoming OOH/UCC and NHS 111 reports in Document Management in EMIS web and reallocating as appropriate.
- Dealing with all EDTs as per practice protocol
- Dealing with telephone enquiries within the practice telephone protocol as necessary.
- Communicating with all staff, patients and outside agencies including the NHS England and hospitals as necessary.
- To attend any necessary training courses/sessions.
- To be aware of Elm House Surgery Health & Safety policies, observing the correct and safe procedures are followed.
- To observe the rule of patient confidentiality and be aware of the Data Protection Act.
