Are you a motivated Health Care Assistant looking for a new challenge?An exciting opportunity has arisen for a dynamic and motivated HCA to join our highly skilled, innovative and rapidly expanding Frailty service
The post holder will be based at Bevan Court, a 30 bedded ward that consists of medically optimised patients, or patients who otherwise attend the trust without the need for ongoing care on an acute medical or surgical ward.
Patients here will be looked after by a multidisciplinary team or nurses, doctors, therapists, social workers, pharmacy and volunteers as part of the frailty service.
The overall aim of the unit will be to provide care and support planning in preparation for hospital discharge, The Frailty Service compromises of:A dedicated Ambulatory unit, which provides rapid Comprehensive Geriatric (CGA) for patients with frailty and frailty syndrome.
A 16 bed in patient Frailty Assessment ward, that benefits from daily Consultant Geriatrician leadership along with a dedicated team of nursing, therapy and social care professionals.
A Community Frailty Service for St Helens CCG that is reactive in order to prevent hospital admission and also provides ongoing management for frail patients in the community.

To work as part of the Nursing Team undertaking household support tasks and direct care tasks as delegated by the Nurse in Charge.
To assist in providing an environment within the ward that is conductive to patient recovery and pleasant for patients and their families.
To accept responsibility for specifically delegated functions within the ward which the Nurse in Charge feels are appropriate for untrained staff to undertake.
To ensure where entries are made in nursing documentation, that these are correct, legible and comprehensive and countersigned by a registered nurse.
To ensure the Health and Safety of patients, relatives and other staff is considered and not prejudiced by the post-holder at any time.
To ensure that all patient information is confidential and is not disclosed to a third party except with permission of the Nurse in Charge.
To ensure that all patient care is delivered with the highest degree of professionalism.
To present an appearance which gives a positive image of the Trust and a manner which is appropriate and which inspires confidence.
