To support with the care of patients on wards, including the Older Person's Rapid Assessment Unit, acute Medicine for Older People wards and AMUs.

This post will particularly be related to managing unwell medical patients with a focus on optimising them for safe and timely discharges.

The job title of the post is Senior Clinical Fellow in Medicine for Older People & Frailty.
The post is paid on the Senior Clinical Fellow scale with on-call salary incorporated within this.

Applicants must have at least 2 years of post-registration training and to have worked at least 6 months post-registration in a medical speciality either pre-or post-registration.

Job Plans will be finalised with post-holders on an individual basis.

Dependent on experience, training needs and department requirement, an element of contracted time may be used for, CPD, research or contributions to departmental administration.

The successful applicant will be encouraged to advance their career in geriatric medicine and willbe supported in obtaining postgraduate qualifications.

They will also be allocated an educational supervisor.

The applicant will also be expected to take part in the Acute on-call rota.


East Lancashire Hospitals NHS Trust is looking to recruit a Senior Clinical Fellow working across our Geriatric wards in both our acute and community hospital sites within the directorate of Medicine for Older People.

This is a full time post, on a fixed term contract basis for 12 months.

Experience of working on busy Medical Wards is essential.

Full GMC registration with a license to practice is required


About the directorate:


The post holder will work in partnership with Consultants, specialty grade and junior medical colleagues, matrons, ward managers, nursing and therapy teams as part of a multi-professional team.


The post-holder will also take part within the Acute on-call rota under the Acute Medicine Directorate completing long days, nights, and weekend shifts as part of an established rota shift pattern.

Within this on-call rota, the post holder will actively participate in senior overview of the management of patients within the acute team, managing their own patient workload with consultants present on the unit 08:00 - 20:30 seven days a week.


MEDICINE FOR OLDER PEOPLE DIRECTORATE
This is an exciting time in Medicine for Older People.

In recent years there has been a national push to ensure that the NHS is "Fit for Frailty" and, in the Northwest, East Lancashire Hospitals Trust is leading the charge.

We were selected as one of the first sites for the Royal College of Physicians "Future Hospitals" project and a lot of changes have been made to our service provision for older people as a result.

These changes include providing "front door" frailty assessment and review in the Emergency Department, enhancing community geriatrics support and care in our community hospitals and creating an Older Persons' Rapid Assessment Unit to ensure safe, personal and effective care for the frail older people that present acutely.

All of these align with our Trust's Improvement Priorities.

You will be joining a dedicated and passionate multidisciplinary team who are committed to making our Frailty Pathway a reference point of excellence in the Northwest and indeed in the country.

Experience of working on busy Medical Wards is essential. Full GMC registration with a licence to practise is required.

Medicine for Older People services are throughout all 5 sites of the Trust with the acute stroke unit and four acute MfOP wards on the Blackburn site and step down and rehabilitation wards on all other four sites.

Duties of the post

The successful applicant is expected to work as a member of the clinical team under the guidance and supervision of consultant geriatrician working in partnership with other members of the medical and multi-disciplinary team, and under consultant supervision and guidance.

They will be based on the acute hospital site but will be part of a wider team supporting the care of our patients in the community both in intermediate care beds and with primary care colleagues.

They will be expected to undertake a range of clinical duties including the daily clinical assessment, diagnosis and treatment of inpatients, providing immediate care to deteriorating patients including initial resuscitation where appropriate, appropriate referrals to other relevant specialist teams where indicated, and supporting safe discharge and follow up.

They will be
expected to follow local and national guidance and to contribute to these.
