Description

Our Neighbourhood Palliative Care Leads are community nurse practitioners providing 7 day palliative and end of life care support and clinical leadership within our Neighbourhood Teams.

They work collaboratively with Neighbourhood Team Case Managers, Neighbourhood Leadership Teams and Specialist Palliative Care Services to increase staff knowledge, skills and confidence, embed best practice and continuously improve the care of patients on Neighbourhood Team caseloads.

The role involves delivering direct clinical care, providing practice based support, advice and staff development and contributing to the delivery of education, training and clinical audit programmes that support the delivery of high quality care.

Leeds has a strong collaborative approach to palliative and end of life care and a key element of this role will be working closely with partners in the wider multidisciplinary team and Leeds Palliative Care Network.
